The HBT is pleased to provide you with the various notices and disclosures related to the health plan (the “Plan”). Please carefully review the information contained in the various notices and disclosures and share them with your covered dependents. In the event of a conflict between the official Plan Documents and these legal notices or any other communication related to the Plan, the official Plan Document will govern.

There are several ways you can easily review the various notices and disclosures. These are described below:

Review the notices on this website and download and print copies.
Visit your employer’s Human Resources office and ask to see copies. Your employer’s Human Resources office will obtain the requested materials and then supply them to you.
Call the insurance carrier at the phone number printed on your personalized member ID card and request that a benefit booklet be mailed or emailed to you.
Call the PAISBOA Health Benefit Trust at 484-580-8844 and request that copies be mailed or emailed to you.
Visit the PAISBOA Health Benefit Trust Office and ask to see copies.
